Registered Nurse Clinical Care Coordinator

*PART TIME TEMP TO PERMANENT POSITION!* *Registered Nurse – Clinical Care Coordinator – Stuart Florida* Evergreen Private Care has an exciting opportunity for a part-time Clinical Care Coordinator.
This Registered Nurse (RN) will complete initial and ongoing assessments to identify the physical, psychosocial and environmental needs of private pay home health clients.
Nurse will develop, initiate and revise the plan of care as needed to ensure quality and continuity of care.
Working closely with the Director of Nursing, clinical team and schedulers, the nurse will monitor assigned cases and help to coordinate services.
Position includes participation in our after-hours clinical on-call rotation which consists of occasional weekend and holiday coverage.
The right candidate will enjoy working amongst a close-knit group of nurses and support staff and will participate in weekly in-office meetings as well as orient new care providers on skills assessments.
RN will provide assistance to clinical team and will work to fill-in care gaps when needed as well as share in workload tasks as assigned.
Participate with care solutions program by providing assistance to clients and their families with various requests.
Position is PART TIME and requires visits to the client’s homes as well as administrative time in the office.
Compensation package: $35-$40/hr, 20-30 hours per week, flexible schedule.
Hours: M-F 12pm-5pm, alternating one late day per week (3pm-8pm) www.
